Centrepoint Chiropractic Clinic is committed to protecting the privacy of your personal and health information. To provide safe and appropriate chiropractic care, we collect information including your name, contact details, date of birth, Medicare, DVA or health fund details where relevant, your health history, and clinical records relating to your assessment and care at the clinic.
Why We Collect It
Your information is collected for the primary purpose of providing chiropractic care. It is also used to manage appointments, communicate with you about your care, process HICAPS health fund claims, bill DVA or Medicare where applicable, and, where you have provided a referral, communicate with your GP or other treating practitioners involved in your care.
How Your Information Is Stored
Patient records are stored securely at the clinic and access is restricted to Dr David Chapman (Chiropractor) and authorised staff. We take reasonable steps to protect your information from misuse, interference, loss, unauthorised access, modification or disclosure.
Disclosure of Your Information
We do not sell or share your personal information for marketing purposes. Your information is only disclosed to third parties with your consent (for example, to your GP under a GP Chronic Condition Management Plan, or to DVA or your health fund for claiming purposes), or where disclosure is required or authorised by law.
Accessing or Correcting Your Information

If you contact us through this website, the details you provide are used only to respond to your enquiry. This website does not use advertising trackers and does not display third-party advertising.
